After, what I think is a mega event, a bunch of previously undiscovered class 25 worlds have been discovered.
In my list of uncolonised worlds, some of these planets are duplicated as per image

Some are listed twice, one three times.
I checked, they do refer to the same planet when I click on each entry.
EDIT: The uncolonised worlds list is also flashing. My colonised world list is fine.

EDIT 2:
Once the worlds have been colonised, the duplicate entries are still showing.
And it looks like there is a uncolonised planet under the colonised planet as per image
