See picture below, however, this shows that the two planets right below mine both have 2 factions on them each. You can see the different icons, one behind the other. This has been in the game since pre-launch. I can include save game if needed and create ticket, however, it can be reproduced on any game that has enough factions to put the number known just over the limit of the diplomacy screen prior to it creating another ring.
